Comprehensive Evaluation
We’ll obtain your doctor’s referral and then review whether your services are covered by insurance or require private pay, so you’ll know any out-of-pocket costs upfront. Next, we come to your home for a thorough interview and use evidence-based assessments to discover your needs and prioritize services—whether that’s fall prevention, driving safety, medication management, sleep support, incontinence care, and daily home management
—to support successful aging in place.

Personalized Plan
Together, we’ll design a personalized plan to build skills and strategies that enhance safety, confidence, and independence. Your plan may include recommendations for practical strategies, affordable equipment, or home modifications to make daily activities easier, reduce fall risk, and support healthy aging through non-medication approaches. We also offer driving rehabilitation to maintain safe mobility—or guidance for planning driving retirement when the time is right.

Caregiver Collaboration
We include caregivers in every step of the aging-in-place process, providing guidance to empower your loved one and maintain independence. We connect you with trusted community resources across multiple areas: elder law, financial guidance, mental health, social engagement, and handyman and contracting services. By integrating these services with our occupational therapy support, we help create a comprehensive plan that keeps your loved one safe, independent, and supported.

Create a safe environment and gain skills to help you stay independent
within the comfort of your home.

Phone Consultation
First, we'll have a complimentary 20 minute phone consultation to meet each other, and answer any questions you have.
At this time we will also collect Medicare B information that you or your loved one is enrolled in.

In-Home
Assessment
We come right to you to develop a plan customized to your needs.
We will assess you (the person), the environment (your home), and your tasks (what matters to you). Included is a full report with recommendations and resources that fit your needs.

Follow up
Follow-up visits to establish adaptive strategies and home exercise programs, train on home modifications and equipment, and provide caregiver training, will be scheduled.
Suppose only a home safety assessment is needed. In that case, it includes a follow-up phone call to see how the new recommendations are working and address any questions.

Kristen's passion is helping people stay in their homes! Over the years, she has seen the strain on families when illness, surgery or disease makes returning home difficult or impossible. She has witnessed the isolation and lack of support seniors can experience even under Assisted Living care. She has helped countless caregivers navigate their loved ones' advancing needs.
These experiences and passions have lead Kristen to develop an innovative, holistic approach to rehabilitation which centers on what is most important to her, clients in their homes.
