Follow up
First, we'll have a complimentary 20 minute phone consultation to meet each other, and answer any questions you have.
At this time, we will also collect your physician and insurance information and discuss the typical process and expectations for the services you are interested in.
We come to you and create a plan that fits your needs.
We assess you, your home, and the daily tasks that matter most. If you need a driving evaluation, we do the clinical testing in your home and the behind-the-wheel testing in the areas where you usually drive. Every service includes a clear report with recommendations and resources tailored to you.
Follow-up visits to work toward your goals, educate, establish adaptive strategies and home exercise programs, train on home modifications and equipment, practice adaptive driving equipment, or provide caregiver training will be scheduled as needed.

Comprehensive Evaluation
We’ll obtain your doctor’s referral and then review whether your services are covered by insurance or require private pay, so you’ll know any out-of-pocket costs upfront. Next, we come to your home for a thorough interview and use evidence-based assessments to identify your needs and prioritize services—whether that’s driving safety, home safety and fall prevention, or daily home management, such as medications—to support successful aging in place.

Personalized Plan
Together, we’ll create a personalized plan centered on what matters to you—building the skills, confidence, and safety needed to stay independent. Your plan may include practical strategies, affordable equipment, or home modifications to make daily life easier and reduce fall risk. We also offer driving rehabilitation to support safe mobility or guidance for planning driving retirement when the time is right.

First, we include caregivers every step of the way, offering guidance that empowers your loved one while supporting independence.
Our team connects you with trusted community partners in areas such as contractors and home maintenance services, in-home services, elder law and estate planning, mental health, and social engagement. Together, we create a coordinated plan that keeps your loved one safe, confident, and supported at home.

Kristen's passion is helping people stay safe and independent in their homes and on the road. Over the years, she has seen the strain on families when illness, surgery, or disease makes returning home difficult—or when driving skills change, creating new challenges for independence. She has helped countless caregivers navigate their loved ones’ advancing needs with compassion and expertise.
These experiences inspired Kristen to develop an innovative, holistic approach to rehabilitation, focusing on what matters most to her clients: maintaining independence at home and planning for safe driving. By assessing driving skills, preparing for driving retirement when needed, improving home safety, and building practical skills, Kristen helps clients continue to engage in the activities they love. Her goal is to make the dream of aging safely and confidently in place a reality—at home and on the road.
